Have your say on Community Hospitals

The NHS is asking patients and carers in Leicestershire for their views on how it should deliver care closer to home and community settings, including five community hospitals in West Leicestershire.
The five community hospitals are:
Coalville Community Hospital
Hinckley and Bosworth Community Hospital
Hinckley and District Community Hospital
Loughborough Community Hospital
Ashby-de-la-Zouch Community Hospital

Translations and large print versions of the questionnaire are also available.
The questionnaire will close on 22 July 2012 so you've still got plenty of time to make sure your views are heard.
If you're keen to find out more about the future of Community Hospitals in Leicestershire, you're welcome to go along to one of two workshops - where you'll also be able to have your say.
July 9th from 7pm to 9pm (registration from 6.30pm) at the Hood Park Leisure Centre, Norfolk Street, Ashby-de-la-Zouch, LE67 1HU

July 16th from 2pm to 4pm (registration from 1.30pm) at the St Francis Community Centre, Tudor Road, Hinckley, LE10 0EQ
